Summary: A 44 year old female presented at the LaserSculpt Clinic in Liverpool with general dissatisfaction in the appearance of her anterior abdominal wall following 4 previous pregnancies.
SculpSure body contouring was performed using 12 diode laser applicators requiring a total treatment time of 75 minutes.
The result measured at 6 weeks was best observed on lateral photography and demonstrates a marked reduction in the distortion of the anterior abdominal skin, a reduction in the fat folds and creases and an overall reduction in abdominal girth.
Presenting complaint: A 44 year old female concerned with the appearance of her post part abdominal wall.
Medical history: Attended the Liverpool LaserSculpt Clinic in March 2017. Following 4 previous pregnancies, she had some cutaneous creasing of the skin overlying the anterior abdomen and had excess anterior abdominal fat.
She was in good health with no other medical conditions to note and took no medication. She had an average diet and kept well hydrated, drinking very little alcohol. A busy mother and career women, she did very little exercise with the exception of regular dog walking.
She had never undergone other cosmetic procedures other than dental braces and had a previous surgical history of an appendicectomy many years earlier. Her menstrual cycle was normal and there was no possibility of pregnancy.
Drug history: None. No known allergies
Examination: Deposits of subcutaneous fat were identified across the anterior abdominal wall affecting both upper and lower abdomen. There was also “love handle” fat bilaterally. Stretch marks were present across the lower abdomen associated with earlier pregnancies and there was significant laxity of the skin over the lower abdomen. There were no hernias, divergent recti, cutaneous lesions of concern of evidence of other contraindications to treatment.
Height was 157cm, weight 52.4 kg with a resulting BMI of 21. The waistline measured at the umbilicus measured 28.5 inches. At the level of the hips it was 32 inches.
Pre-procedural photography was performed at 45 degree angles sequentially to map the entire torso in a neutral position and in exhalation with the arms above the shoulders to exclude them from view.
Treatment: Three 25 minute SculpSure treatments were prescribed using 4 diode lasers for each. The first was applied to the upper abdomen in a 2x2 grid, the upper two in portrait and the lower two in landscape orientation. The second 25 minute treatment was applied across the lower abdomen, targeting the area of maximum deposit and skin laxity, using four in-line portrait diode lasers. The last treatment targeted the lateral deposits using 2 diodes applied to each side in portrait orientation. Following the four minute build cycle a stepped increase in laser power was delivered until full power was achieved in all treatments by 8 minutes and for the remainder of the treatment period. The client had no significant side effects but did experience heating and cooling sensations and a feeling of either pinching or pressure depending on the area being treated.
Following the procedure the client was advised to keep well hydrated and to lightly massage the areas treated twice daily for 5 minutes in line with the standard laser manufacturer’s advice.
25 Minute Upper Abdominal SculpSure Treatment and Before and After Results at LaserSculpt Clinic
Results: The client returned to the LaserSculpt Clinic 6 weeks after the procedure. Evaluation photography was performed in identical positions to the pre-procedural evaluation. There had been a significant reduction in the appearance of the depth of abdominal fat. Deeper creasing across the lower abdomen also appeared improved. The client reported skin tightening across the lower abdomen. There was no change to the appearance of any stretch marks and no new lesions were identified.
The client reported no change in diet or exercise but there was a decrease in weight noted of 2.9kg to 49.5kg with a decrease in BMI from 21 to 20.1. The waistline measured at the umbilicus measured 26.5 inches and the hip measurement remained unchanged at 32 inches. The client was delighted with the results and reported increased confidence.
Conclusion: SculpSure laser body contouring can reduce the appearance of subcutaneous fat in selective areas. The device is FDA approved for use on abdomen, flanks, backs and extremities. Procedural times are 25 minutes and results are visible typically 6 weeks after a single session of treatment. SculpSure does not treat visceral fat which lies within the abdomen.
